Title: Do Calcium Channel Blockers play a role in preventing Hypertensive Encephalopathy? A- Study aim To identify the risk factors for hypertensive encephalopathy amongst patients seen in the Emergency Department (ED) and assess whether calcium channel blockers play a role in preventing hypertensive encephalopathy. B- Background and significance Hypertension is a very common problem, affecting 1 billion people worldwide, with 50 million cases in the United States, and one third of cases going undiagnosed. 1% of the cases will experience a hypertensive emergency in their life. (1,2) Although 1% is not a high number, the complications for that percentage are very costly and disable things like End Stage renal disease, Stroke, andâ⬠¦show more contentâ⬠¦The symptoms are usually reversible with adequate and prompt control of the blood pressure. (6) The aim of the study is to identify if patients on CCB have less of a chance of developing hypertensive encephalopathy than those treated with different antihypertensive medications. Calcium channel blockers are a group of medications used to treat Hypertension, and also used as prophylactic treatments for migraines. They interfere with calcium influx to the arterial wall, which cause arterial dilatation and relaxation of the arteries and hence lowers blood pressure. Nicardipine is one of the CCBââ¬â¢s used commonly in treating stroke patients with very high and uncontrolled hypertension. (6) ââ¬Å"Nicardipine crosses the blood brain barrier which binds to the calcium-channels of the L-type, acting primarily at the level of the hippocampus (7). Intravenous Nicardipine has been shown to reduce both cardiac and cerebral ischemia(8).â⬠We want to observe if the patient treated with calcium channel blockers medication as outpatient home medication has less hypertensive encephalopathy complications. Moreover, we went to test if calcium channel blockers play any protective role against patients developing hypertensive encephalopathy. C- Innovation of this study There is multiple studies about the use of CCBââ¬â¢s in treating Hypertensive emergencies, one study in 1985 shows that CCBââ¬â¢s and specifically the Nicardipine could be used as first line drug in management

When writing prose, one expects either first or third person, but not both which OCallaghan does in his goth novella, Nothing on Earth. Lodge (p. 88, 1988) states that writing in the two tenses can disorientate the reader and disrupt logical relationships between the concepts or entities or events. Typically, the genre is flamboyant and disjointed, with grim content which is certainly true of OCallaghans novella. The reader may feel disengaged by several nameless characters: the girl, a cleaner, an elderly priest (a pivotal character and narrator), and his brother. While the novel may not be a dialogue with the author (Fludernik, 2001), readers could find their quest for the origin of the text, (Dawson, 2013) and interactional experience, (ibid) puzzling. Although OCallaghans voice remains invisible (p. 45, Alvarez, 2005), the reader is invited to imagine hearing a voice on a page speak out to .. . grab your attention, (Utley, 2015) which the narrators voice achieves resolving any polemic of voice. OCallaghan, however, entices readers, embroidering the books fabric with florid language: the girls [English] freeze-dried taken out of the vacuum packing (p. 11), drifted through those rooms with invisibilities weightlessness (p. 45) or slid into her sandals (p. 69). Voice give[s] the impression of speaking naturally is persuasive, artificiality is the contrary; for our hearers are prejudiced, Elbow (2007). An obvious naturalness exists between Martina and her niece, the girl, as they chat in the garden (p. 60). Martina asks the girl what Mutti (as the girl calls her mother, Helen, not her real name (p. 21)), said about her darling sister [Martina] (p. 60). The girl replies Martina loved being gawked at (ibid); softer vowels and consonants accentuate the hard, gutteral voiced plosive of /g/ and /k/ in gawked in this paragraph. Martina and the girl sunbathe as if it is work (p. 63). Using Martinas phone, the girl sets a timer alert, records herself singing Everybody turn, and to end their sunbathing ritual sets the timer to play Everybody turn! Everybody turn! (p. 63). Disappearance of the girls parents, Helen (p. ), Paul (p. 109), and the girl herself (p. 142) and related police investigations. Paul rejects police suspicions that Helen went to London (p. 53). When Paul was made redundant, the girl was waiting outside in pyjamas (p. 81). He exclaims: What the hell is going on? The girl felt safer outside, .. . [Martine]s not in her room(ibid). Paul calls the property developer, Flood, asks for Marcus, their security guard (p. 84). Reluctantly Flood admits Marcus went to Reading weeks ago (p. 85). Ipso facto no site security and Martina had doll[ed] herself up to visit an empty caravan (ibid). One of many far-fetched scenarios is the girls father, Pauls, disappearance waiting for the girls freshly baked flapjacks which she had said would be ready in ten minutes (p. 108). The girl calls her father who doesnt reply. She goes through the rooms but cannot find him. Then, [t]he kitchen started bleating:(p. 109) Everybody turn, â⬠¦ Everybody turnâ⬠¦, (ibid). Both the front and back door were locked from the inside. The keys rattle in her fathers jacket pocket (ibid). It harkens back to sunbathing with Martina; the girl recorded herself singing Everybody turn, â⬠¦ Everybody turn. (p.63), pose more unanswered questions: Where did Paul go? How had he left a house which was locked on the inside, with the keys in his jacket pocket in the house? Is the bleating kitchen the mobile phone?Language describing the girls reactions convey a demented, panic-stricken flight. Phrases signalling her desperation include: sprinted without breathing or stopping and she hammered [on the door] (p. 11). A traumatised child, what drove her to [try] very hard not to start crying (p. 11) while explaining her desolation to the priest whose front door she had pounded. Attempts to answer such questions is an academic exercise in futility. As the novella begins, so it ends. A pattern of people, services and things appear or disappear. The girl, on encountering the priest, says, one minute he [her Papa] is behind you. And next time he was gone. (p. 11); the girls mother, Helen goes out and didnt return, (p. 51); cement mixer, breeze blocks, soil, wheels, doors, disappeared (p. 69); [a]fter the water, the money ran out. . .. line went dead (p. 105); Paul shaking hands with the Poles at number 3 (p. 92); Martinas unreported disappearance a month ago, (p. 135), demons, noises and Poles in number three which the Priest knew .. . remained vacant (p. 138). The novella concludes in the only way it can; the brother from the States stop[s] anwering the priests calls (p. 173). In interviews, OCallaghan says life is uncertain and inconclusive and everything we know about the book [its oddities and anomalies] are in the book. We can only know what is in the book. If it is not in the book we cannot know it (ibid), a truism of fiction, but quintessential to OCallaghans novella.

That ref just sucked!â⬠I sat alone in the second row of our minivan, shivering slightly in my basketball singlet and shorts. I should have worn a jacket to combat the January chill, but being a stubborn 10-year-old, I had insisted that I didnââ¬â¢t need one. Despite the freezing cold walk from the parking lot to the game, I had played well. Extremely well. Except the stupid referee kept making the wrong calls! I was knocked to the ground by an overly aggressive forward, and a foul wasnââ¬â¢t even called. When I barely brushed the forwardââ¬â¢s shoulder on the next play, the whistle was blown on me! The injustice was unbearable. It was obvious that the referee was either in serious need of a pair of glasses or wanted the other team to win, yet when I tried to express my indignation during the car ride home, I noticed that my dadââ¬â¢s expression grew sterner in the rearview mirror. Were there a couple of calls that didnââ¬â¢t go your way?â⬠While he was taking a breath, I snuck in an angry, inaudible, ââ¬Å"Yup.â⬠ââ¬Å"Yeah, there were a couple,â⬠he conceded, answering his own rhetorical question. ââ¬Å"But there were also a couple of calls that did go your way that shouldnââ¬â¢t have, and you know what? Right now, the other team is whining about those calls.â⬠Stubbornly refusing to meet my dadââ¬â¢s eyes in the rearview mirror, I thought about what he had said. Although at first it had seemed perfectly obvious that the referee was secretly working for the other team, I supposed that he did make a few calls that went our way. I truly hated to admit it, but my dad was probably right: it wasnââ¬â¢t worth getting upset about a couple of wrong calls. I no longer play basketball, but my dadââ¬â¢s lecture has stuck with me. When I race, I always take the time to congratulate my opponents, no matter who wins. As a result of my good sportsmanship, Iââ¬â¢ve become friends with my rivals and look forward to seeing them at each meet. Watching basketball and soccer games, I donââ¬â¢t heckle the players or referees. When the rest of the fans are yelling, ââ¬Å"Warm the bus up!â⬠to the losing team, I try to focus on the game. And I certainly donââ¬â¢t spend the drive home whining about how awful the referees were. I thought that my dad was done with his talk, so as we pulled into our driveway, I stole a glace at him in the rearview mirror. Rookie mistake. My dadââ¬â¢s hazel eyes were looking piercingly back at me, and he added one final thought. ââ¬Å"And nobody likes to be around a whiner.ââ¬

Pornography and Women Theory essays Constitutionally protected speech that is Clearly sexual abuse is discriminating and unconstitutional, therefore, must be restricted speech. Catherine A. MacKinnon, in her book Only Words gives persuading evidence that pornography subordinates women as a group through sexual abuse. She says Protecting pornography means protecting sexual abuse as speech, at the same time that both pornography and its protection have deprived women of speech, especially speech against pornography (MacKinnon, 9). MacKinnon argues this bye explaining defamation and discrimination, racial and sexual harassment, and equality and Women are sexually abused for the making of pornography. Torture, rape, hot wax dripping over nipples, and murdering women are the tools to produce a product of evil. Literature is the description of these crimes against humanity (emphasized) and cameras are On the assumption that words have only a referential relation to reality, pornography is defended as only words-even when it is pictures women had to directly used to make, even when the means of writing are womens bodies, even when a women is destroyed in order to say it or show it or because it was said or shown. However, assuming words are only a partial relation to reality would mean we would have to reconsider what reality is. Our wedding vows such as I do would be meaningless and a jury could never return a verdict that is only partial to reality. These words are treated as the institutions and practices they constitute, rather than as expressions of the idea they embody (Mackinnon, 13) Therefore, if these words of pornography are only words, dont they institutionalize rape? Since pornography is rape on women. Pornography is protected by the First Amendment as free speech, but why? Becaus ...

I started out by rewriting my entire first body paragraph which had a lot to do with coffee and its crucial role to the United States, and the South American economy. I had to delete the majority of this paragraph and re-read all my sources. I rewrote this paragraph and stated out in a lot of detail the importance and beneficial role that coffee had to play in the American economy. This time I did so in my own words. I elaborated in all the details such as how coffee affected small coffee houses and how it contributed in the making of worldwide jobs. The organization in this essay wasn 't so great so i ended up organizing it much better. I started out by first talking about how coffee benefited the U.S economy, and then i moved on into the effects it had on the South American economy. ce on how caffeine can be a risk reducer towards type II diabetes and liver cancer was a walk in the park metaphorically speaking. Another strength of mine is my wordplay. While this can also be a big w eakness because at times I can write some really awkward sentences it can also bring about some creativity on how I state things out. I used this to my advantage and used it on my first essay, the explanatory synthesis essay. Concluding this essay as a writer i feel more knowledgeable and in a sense my skills have vastly improved. I know how to use sources and scientific articles. I 've also learned how to cite sources and write a good bibliography. My knowledge has also expanded to the point where i now know how to use scholarly journals. As a growing writer I feel as if I have a lot more to learn in the near future and i feel like i have a lot of growing left to do.
